The undersigned is authorized to commit the organization submitting this job order and agrees to the following: In the event that the organization hires a full- or part-time person referred to us by SER (“referral”), we shall pay SER a Placement Fee in an amount equal to two weeks of the referral's average gross salary or wage. If the position has been advertised as temporary, we shall pay SER an amount equal to 10% of the total gross salary paid to the referral during his/her full period of employment. The minimum placement fee is $100.00. Approximately 30 days after the referral’s commencement of employment with you, SER will contact you to confirm that the employee is performing satisfactorily. (The timing of this may vary if you have hired the employee for a temporary position.) If you are satisfied with the employee’s performance and SER’s services, we will send you an invoice for the Placement Fee. If you are not satisfied, no Placement Fee will be charged with respect to that employee. All fees are payable within 15 days of the invoice date. It is understood that SER will take collection action if payment is not received within 30 days after we send you an invoice.
